The makers mark is on the base of the jug.

There doesn't seem to be a date of model number on the jug.

Width: Approx. 8.5"

Rim: Approx. 3" x 5"

Base: Approx. 4.5" x 4"

Please note that this item was donated used so it may show some signs of age and use, but overall it does seem to still be in generally good condition.
